Discipline Policy
Unacceptable behaviors:
1. Physical contact-hitting, kicking, biting, pulling hair, choking, pinching,
tackling, etc..
2. Foul Language.
3. Blatant disrespect to adults/counselors.
4. Destruction of property and/or equipment.
5. Any Deviation from Pool and General safety rules.
The children are not to hurt another person or child physically or verbally. If
a child is hurt in some way, they should walk away and ask for help from a
counselor. When a problem does occur the staff shall listen to both sides of the
argument and discuss with both parties there mistakes (if applicable) and what
choices should have been made. If a child is too hot for a conversation we will
separate the child from the group with a “ quiet cool off” next to a counselor.
Discipline procedures will include loss of privileges, clean up duty, and a
written conduct from written to the parents. If additional problems occur with
the child throughout the day, parents will be called and asked to speak with the
child. . If this does not work the parent will be asked to pick up the child.
Injuries: The sport camp is very active, and as such, injuries can and do
occur. In the case of injury, we will call the parent and notify them. The
parent can then decide if further treatment is necessary.
Illness: When a child is not feeling well, we assess the situation and
then keep the child in a quiet place away from the other children. If the child
requests or if the problem persists we will call the child’s parents. If the
child is running a fever, vomiting, or diarrhea, parents will be called to take
the child home. Please do not send your child if he/she has ANY contagious
illness or infection.
After School Homework: Children will have homework time shortly after
they arrive. Counselors will do their best to assist children in homework time,
but homework should be reviewed at home.

Waiver and Indemnity Agreement
The undersigned, intending to be legally bound hereby and in consideration of
the use of facilities of the Apollo Beach Racquet and Fitness Club, their
employees, agents and contractors from any and all liability or claims for
bodily injury to the above named participant and loss or damage to personal
property with respect to any and all claims, suites and losses(including
property damage) arising out of the use of facilities of the Apollo Beach
Racquet and Fitness Club. I further understand that accidents occasionally occur
during participation in sports and physical activities. Knowing the risks,
nevertheless, I hereby agree to assume those risks and to release and hold
harmless the Apollo Beach Racquet and Fitness Club, it’s employees and agents. I
hereby acknowledge that I have read and understand the waiver and indemnity
agreement for the above-mentioned parties.

Authorization for Emergency Medical Treatment
If my child (child’s full name)______________________________should become ill
or injured the Apollo Beach Racquet and Fitness Club, I understand that the ABR&FC
will: 1.) contact me immediately and 2.) contact the person(s) I have designated
if I cannot be reached. Should ABR&FC be unable to reach me and/or the person(s)
designated, they are authorized to contact my child’s physician and/or arrange
for immediate medical treatment. The physician and/or medical facility are
authorized to administer emergency medical treatment necessary to ensure health
and safety of my child.

Hillsborough County Ordinance90-38 or 92-20 requires that all parents must
receive a copy of the child care facility Know your Child’s Day Care Center. The
parents or legal guardians signature verifies receipt of the child care
brochure.
